We are conducting a survey of
our county to learn more about the health and quality of life in Lee County. The Lee County Health Department, LeeCAN and Lee
County United Way will use the results of this survey to help address the major
health and community issues in our county.
The survey is completely
voluntary, and it should take no longer than 20 minutes to complete. Your answers will be completely confidential.
